One note on health care. The markets are concentrated because of government. For example, in my State of Washington, in the ’90′s, we passed a reg declaring that insurers must cover alternative medicine. Insurers fled the State, leaving only 4!

Health care is a monopoly, though. Insurance is not. The AMA has a monopoly, and has had since 1849, when Congress gave it to them. They drove all their competitors out of business within five years. The village wise woman with her home remedies, the Chinese herbalist, et al, were all out of business. This left us with the most expensive, ineffective options: drugs and surgery.

This is why health care is so expensive, becuase there are no options. It is a monopoly. The problem could be solved by (among other things) taking back the AMA’s authority, thus breaking the monopoly.
