The reason is that the reactor needs to be designed from the beginning with this capability in mind. Essentially, there is a port into the reactor designed for easy access. It is a significant added expense, so if all you want to do is generate electric power, you won’t bother.

Clearly the world needs more reactors with the capability so that nuclear medicine can continue even if one (or two) is out of service.
